Hospital management software price in India in 2026

What a cloud HMIS costs a small or mid-size Indian hospital in 2026, how vendors structure pricing, what must be included, and the add-ons to ask about.

In 2026 a cloud hospital management system for a small or mid-size Indian hospital costs from a few tens of thousands of rupees a year to a few lakh, depending on bed count, users and what the vendor treats as an add-on. Healthixio publishes its prices: ₹40,000 in the first year and ₹20,000 a year after that for a clinic or nursing home up to 20 beds, and ₹70,000 then ₹29,000 for a hospital up to 50 beds, ex-GST, with onboarding, migration, training and support included.

Most vendors do not publish a number, which makes comparison hard. This article explains the pricing models you will meet, uses our own published figures as a reference point, and lists what a quotation must include before you can compare two of them.

All prices are ex-GST. Every plan includes onboarding, data migration from your current system, staff training and 24/7 support, and the ABDM module is part of the product rather than an add-on. The current figures are always on the Healthixio page and on healthixio.com; if a number changes, it changes there first.

What must be included in the price?

A quotation is only comparable if the scope is. Before you compare numbers, confirm that each of these is inside the price and not on a separate line you will discover later.

  • Onboarding and configuration: your departments, wards, tariff, doctors, letterheads.
  • Data migration: patient master, pending dues, pharmacy stock with batches. Without this you run two systems for a year.
  • Training for every role, and retraining when staff change.
  • Support with a stated response time, including nights and Sundays, because hospitals do not close.
  • ABDM: ABHA creation, HFR and HPR registration, record linking. This should be in the product, not sold as a module.
  • Updates, including the ones forced by NHA, GST or WhatsApp platform changes.
  • Backups, and a written statement of where the data is stored.

What are the hidden costs to ask about?

  • Implementation or setup fee charged on top of year one.
  • Per-branch or per-location charges if you open a second unit.
  • Storage limits for scanned documents and images, with a charge beyond them.
  • Report customisation billed by the hour.
  • Training visits beyond the first, charged per day plus travel.
  • Renewal escalation: a percentage increase each year that is not in the quote.
  • Third-party charges that pass through you: SMS, WhatsApp per-message fees from Meta, payment gateway fees. These are legitimate, but they should be listed, not hidden.
  • Exit: a fee for a data export, or no export at all. Ask for the format in writing before you sign.

How should you compare two quotations?

  1. Ask for both quotes ex-GST. GST is added on the invoice at the rate for software services published on the GST portal, and comparing one inclusive and one exclusive figure is a common mistake.
  2. Add three years together: year one plus two renewals. A cheap first year with an expensive renewal is not cheap.
  3. Match the scope using the two lists above. If a module or a service is missing from one quote, add the vendor's price for it before comparing.
  4. Add hardware. On-premise quotes need a server, a UPS and someone to look after them; cloud quotes need reliable internet and a backup connection.
  5. Add your own staff time. A system that takes three months to go live costs you three months of double entry.

Frequently asked questions

Why does the first year cost more than the renewal?

The first year includes the work that happens once: configuring the hospital, migrating data, training staff and being on site for go-live. Renewals cover hosting, updates, backups and support. Publishing both numbers matters, because a vendor that publishes year one and negotiates year two has not really published a price.

Is a one-time licence cheaper than a subscription over five years?

Usually not once you add the server, its replacement, the AMC, backup hardware and the person who maintains it. A one-time licence also tends to freeze the product at the version you bought, so ABDM and GST changes arrive as paid upgrades. Do the five-year sum on paper before deciding; the answer surprises most owners.

Do published Healthixio prices include GST?

No. The prices are ex-GST and are labelled that way on every page, so that the figure you compare is the figure on the invoice before tax. GST is added at the applicable rate for software services. Whether your hospital can claim it as input credit depends on your GST position, so ask your accountant.
