HFR and HPR registration for hospitals and doctors, step by step

HFR and HPR registration means listing your hospital in the Health Facility Registry and your clinicians in the Healthcare Professionals Registry, both run by the National Health Authority under ABDM. Both are free, both are done online on the National Healthcare Providers Registry portal, and both end with an ID that your HMIS uses to tag every record it links to a patient's ABHA.

Without an HFR ID, nothing your hospital generates can be linked under your name. Without HPR IDs, the doctor on a shared prescription is an unverified string of text. So these two registrations are the first practical step in ABDM, ahead of any software feature.

Why do you need both registries?

The HFR answers the question 'which facility created this record'. The HPR answers 'which clinician'. ABDM stamps both onto every document that moves through it, so that a doctor in another city reading your discharge summary can see that it came from a registered hospital and a registered doctor. The registries also feed government dashboards and scheme empanelments, which is why an increasing number of forms ask for the IDs. The exact fields on the portal change from time to time, but if you have this list on the desk, the form is a single sitting.

How do you register the facility in the HFR?

1. Sign in to the registry portal as the facility manager. The login uses a mobile OTP or an ABHA.
2. Choose to register a new facility and search first, to make sure the hospital has not already been entered by someone else. Duplicates are the most common problem.
3. Enter the basic details: name, address, PIN code, and pin the location on the map. Zoom in until the pin is on your building, not on the locality.
4. Enter ownership, facility type, systems of medicine, specialities, services and bed count.
5. Upload the registration certificate if you have one, and enter the contact details that patients and verifiers can use.
6. Review and submit. The entry goes to a district or state verifier. Verification can take days or weeks, so check the status and follow up.
7. When approved, note the HFR ID and enter it in your HMIS. In Healthixio the registration is done from inside the software during onboarding, and the ID is stored automatically.

How does a clinician register in the HPR?

1. The doctor registers themselves; the hospital cannot do it on their behalf. Start on the same registry portal and choose the professional registration.
2. Verify identity with Aadhaar OTP or an existing ABHA. The name and photograph come from Aadhaar.
3. Enter the professional category, system of medicine, qualification, and the council name and registration number.
4. Upload the degree and council certificates.
5. Enter current work details and select your hospital from the facility search. This is what links the doctor to your HFR entry.
6. Submit. The entry is verified against council records. When approved the doctor receives an HPR ID, which your HMIS stores against the doctor's user account.

What goes wrong, and how do you avoid it?

• Name mismatch. The facility name on the form does not match the registration certificate, and the verifier rejects it. Copy the certificate exactly.
• Wrong map pin. The pin sits on the town centre instead of the hospital. Zoom in before you drop it.
• Two facility entries. Someone registered the hospital years ago with an old mobile number. Search before you create, and reclaim the old entry rather than adding a second.
• The manager leaves. The login is tied to a person's mobile. Register the entry with a number the hospital controls, and record who holds it.
• Doctors who never finish. Visiting consultants start the HPR form and stop at the certificate upload. Give them a checklist and a deadline.
• Verification stuck. If nothing moves for weeks, contact the district or state verifier through the portal's help route, with your reference number.

Frequently asked questions

Can my software vendor do the HFR registration for me?

The vendor can prepare and submit the facility entry with you, and an ABDM-integrated HMIS can do it from inside the software. What the vendor cannot do is register your doctors, because HPR registration is tied to each clinician's own Aadhaar. Ask whether HFR support is part of onboarding; in Healthixio it is.

Does a visiting consultant need a separate HPR ID for each hospital?

No. A clinician has one HPR ID and links it to each facility where they practise. The consultant registers once and then adds your hospital from the facility search. Your HMIS stores the same HPR ID against their user account, so records they sign at your hospital carry both your HFR ID and their HPR ID.

Is there a fee for HFR or HPR registration?

No. Both registries are free, and registration is done on the government portal. If anyone asks you for a registration fee, they are charging for their time, not for the registry. Your HMIS vendor may include the work in onboarding, but the National Health Authority does not charge for the entry or the ID.
